If you host multiple domains within your web hosting account, you will need to select the domain you are creating the new address for from the pull down menu provided.

First, enter in the username part of your email address.

Next, enter in a password to use for this email address.  You will need to enter the password twice to confirm. 

NOTE:  If you are creating a FORWARDING Account, these types do not actually create a mailbox but rather forward all mail to the chosen address. This will not require a password to be chosen.
